A Provider is a self-employed person or agency who directly provides services to people with developmental disabilities. All providers must be certified through the Ohio Department of Developmental Disabilities (DODD) before they can provide any services.
Becoming an Independent Provider
Prior to becoming a provider, you will need to complete a background check as part of your application packet.
In addition, you must have 8 hours of initial training and have a current CPR/First Aid certification. You can take the 8-hour initial training online at DODD's MyLearning, follow the step by step guide to get started in MyLearning HERE.

For any service provided; providers must have documentation of that service. Each service has its own documentation requirements, which can be found within the rule for each service found HERE.
Providers can determine how they would like their documentation to appear, but it MUST contain all the required elements addressed in rule.

UI/MUI GUIDANCE
Providers should notify the appropriate parties if an Unusual Incident (UI) / Major Unusual Incident (MUI) occurs.
The following MUI categories require reporting within 4 hours: Accidental or suspicious death; Exploitation; Misappropriation; Neglect; Peer-to-Peer Act; Physical Abuse; Prohibited Sexual Relations; Verbal Abuse & Inquiries from the Media Regarding a MUI.
All other MUI’s and UI’s should be reported as soon as possible or within 24 hours.
To report an Incident contact: (937) 382-7519 Monday through Friday 8:00 a.m. until 4:00 p.m. After hours, on weekends, and on Holidays call: (937) 725-5074.
Anytime a UI/MUI occurs, you must document it on an Incident Reporting Form. Submit the completed Incident Report via email to irsubmissions@clintondd.org.
You are required to maintain a monthly Monthly UI Log and submit it to the Provider Support Coordinator even if you have no incidents.
An MUI Annual Analysis must be completed by 1/31 and submitted to the County Board by 2/28 even if there are no MUI's. A courtesy email reminder will be sent to Providers on our email distribution list about the analysis and its due date, as well as a form that you can use to fill out. However, it remains the Providers responsibility to complete these forms and submit it, regardless of receiving any reminders.PROVIDER RESOURCES
