Flu vaccine is available at the CCHD office at 111 S. Nelson Ave., Suite 1 during regular clinic hours or if a nurse is in the building. Please call 937-382-7221 in advance if you need to come at a time other than regular clinic hours.

 Birth and Death Certificates

Certified copies of births certificates are available for those who were born in Ohio after 1908.  Death certificates are available for those who died in Clinton County after 1908.

Public Health Nursing

 

Immunization clinics are canceled on November 23 and 28 and on December 19, 26 and 28. Call with any questions.

The Public Health Nursing division services include: Immunizations and
Communicable Disease Control, Preventable Health Services, Charlie Fischer Society, School Nursing program, Family Health Services  and Child Care services.

Safety Seat Program

We have car seats available for eligible families. Call to see if you qualify. (937) 382-3829

This program offers child safety car seat and booster inspections, installation guidance, product recall checks, informative information, and has safety seats available to eligible low income families through the Ohio Buckles Buckeyes program.

WIC (Women, Infants, Children)

WIC Division Provides healthy foods and nutrition education for pregnant women
and children under 5 years old. Other services include WIC Foods, Breastfeeding.
