This service is provided in the home or community to assist with daily living skills and community integration provided by an Agency Provider.
This service is provided in the home or community to assist with daily living skills and community integration provided by an Independent Provider.
Enables individuals to access waiver services and travel to community services, activities and other resources.
Provided separate from any home or facility in which an individual resides, focus on non-work activites and include: assessments; personal care; skill reinforcement; training in self determination; recreation and leisure.
Enables waiver participants to obtain transportation to access Adult Day Support, Voc Hab, Supported Employment – Enclave and Supported Employment – Community waiver services, as specified by the ISP.
Environmental accessibility adaptations to structural elements of the interior/exterior of an individual’s home. Services must not be otherwise available through other funding. Must be suitable to increase independence; avoid institutionalization and reduce the need for human assistance. Does not include changes that are of general utility, adding to the square footage of the home, or same modifications with the same calendar year, unless documented need.
Services requiring the skills of an RN or LPN under the supervision of an RN. May include personal care aid services provided incidental to waiver nursing service. Requires a plan of care signed and dated by treating physician.
Services that assist the individual with activities of daily living (ADLs) and instrumental activities of daily living (IADLs): Hygiene, eating, mobility, etc.; General homemaking activities/chores; Bill paying/correspondence; Transporting/accompanying individual to activities; “Running errands on behalf of the individual”; Assisting with self-administration of medication.
Services delivered to an individual in an out-of-home setting to allow respite for caregivers normally providing care. Must include: an overnight stay; available nursing services; personal care aide services; three meals/day that meet individual’s dietary requirements.
