Birth and Death Certificates

Clinton County Health Department can now issue birth certificates for anyone born in the State of Ohio.  Available records are 1908 to present.  This change does not apply to death certificates in which case the county where the death occurred will have to be contacted.  All documents prior to 1908 are located at the Clinton County Records and Archives Center, 111 S. Nelson Avenue, Suite 3 in Wilmington, Ohio; phone: (937) 383-3271.

Ohio is an “open state” which means vital records (both births and deaths) are considered public documents by the State of Ohio.  This means that anyone who can submit the basic facts of a certificate may request a certified copy.  Birth and death records are recorded in the county in which the event occurs regardless of the person’s residence.

Vital Statistics Office Hours are Monday through Thursday, 7:30 a.m. – 4:30 p.m., Friday, 7:30 a.m. – 11:30 a.m. and 12:30 p.m. – 4:30 p.m., excluding holidays.

You will be asked to fill out an application for a birth certificate or death certificate for our records. The only information required is the person’s name and date of birth (or date of death). Funeral Home death certificate request form:FUNERAL HOME APPLICATION FOR DEATH RECORD.

Please send a letter or (application for a birth certificate or a death certificate ) to the above address stating the name at birth (or death) of the person you need a certificate for, their date of birth (or death), a contact phone number where you can be reached, and a self-addressed, stamped envelope.  The requested information is processed and sent out the next day of receipt.

The cost per certificate, which includes a raised seal and registrar’s stamp, is $23.00.  We accept all major credit cards, cash, business checks (in state only), and money orders.  We do not accept personal checks. Submission of a check or money order can be made out to CCHD, an acronym of Clinton County Health Department.
